If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Financial Policy and Payment Options
Our office has prided itself for almost six decades on providing exceptional service, comfort and treatment for our patients. In keeping with this philosophy, we strive to provide our patients with payment options that will allow them to never sacrifice their dental health because of an inability to take care of their financial obligations. Payment is expected at the time services are performed and is accepted in the form of cash, check or credit card. Consideration is given to our seniors as well as those paying in full prior to treatment. We also offer an interest free payment plan provided by the nations largest patient payment program called Care Credit. With Care Credit you pay no up front costs, no pre-payment penalties, and no fees. If further information is desired, please click on the link below or call our office. Please call us if your treatment requires more comprehensive dental work and you want to discuss financial arrangements. Financial arrangements can be made with our treatment coordinators.
If you are fortunate enough to have an employer sponsored dental benefits plan, congratulations! Your employer has contracted with an insurance company to provide you with a wonderful benefit. Our administrative staff has many decades of experience in dealing with many if not most insurance plans. They will always do their best to help you receive the maximum benefit to which you are entitled. As a courtesy to you we will help you process all your insurance claims. Unless prior arrangements have been made, we ask that you pay the deductible and co-payment, which is the estimated amount not covered by your insurance company, at the time we provide service to you. We must emphasize this is an estimate only as insurance companies have a wide variety of rules, plan limitations and exclusions that our office may not be aware of.
